Renewal forms are sent out annually on April 15th. All
licenses shall be renewed annually by June 30th, to avoid delinquent renewal
fees. Initial licenses issued during the last quarter of the fiscal year
(April, May, and June), will not be required to renew during that fiscal
year.
AUDIOLOGISTS
-- Renewals packets that were mailed on April 15, 2008 included the wrong
Memorandum which was addressed to Provisional and Restricted Audiologists.

Online Renewals
All full Speech-Language Pathologists and Audiologists may
renew online! There is an additional $3.00 charge to renew online; however,
identification cards will be received within one week.
Mailed Renewals
Renewals that are mailed to the Board office are usually
processed within six weeks. If you wish to confirm that we have received
your renewal application, please send your renewal via certified mail.
Continuing Education
Licensees shall participate in continuing professional
education activities of at least ten clock hours for each license period,
July 1 through June 30. Audiologists who hold hearing aid dispensing are
required to obtain at least three of the ten hours in hearing aid
dispensing.
Audits
Five percent of our licensees will be audited each year.
Audits are randomly selected and will be notified by certified mail.
Licensees who are audited are required to show proof of their continuing
education obtained from July 1 through June 30 for that fiscal year.
Retired Status
Retired Status is
available to anyone who has not worked in the field of Speech-Language
Pathology and/or Audiology for the fiscal year (July 1 through June 30).
Retired Status allows you to keep your license current, but retire your
continuing education for that fiscal year. The retired licensee may either
submit the required five hours of continuing education each year he/she
claims retired status or may submit all of the hours the year he/she returns
to work in the profession.
Even if you have
worked in the profession only one hour, you are not eligible to claim
Retired Status.
If you wish to
claim retired status for the year, you must fill out a Renewal Application
Form, submit the renewal fee, and complete the Retired Status Affidavit on
the back of the Continuing Education Report Form.
In order to resume
the practice of Speech-Language Pathology and/or Audiology, the licensee
must submit proof of completion of five clock hours of continuing education
in the area of licensure for each year that retired status was maintained
(maximum of 25 hours).
Delinquent Renewals
Delinquent renewals will be accepted by the board through
October 31, provided the delinquent renewal fee is paid in accordance with §
119.C and D, and the continuing education summary form is submitted.
A licensee whose license lapsed on November 1, and applies
to reinstate prior to the following June 30, is required to submit a
completed application, proof of continuing education, initial license fee
and delinquent renewal fee in accordance with § 119.A and D and § 123.
Licensees who allow their license to lapse shall submit
documentation of completion of five clock hours of continuing education
(maximum of 25 hours) in the area of licensure for each year that the
license has lapsed in addition to meeting the license requirements
enumerated in R.S. 37:2650, et seq.
