All name and/or address changes must be submitted to the Board office. This can be done by Name/Address change online form, fax to (225) 756-3472, or by mail to 18550 Highland Road, Suite B, Baton Rouge, LA 70809.
If you need to obtain Verification of Licensure for another state, please submit this request in writing to the Board office with a check in the amount of $5.00. Verifications are usually sent out within two to three business days and can only be sent via first class mail. Verification of out-of-state licensure sent to the LBESPA office must contain a board seal.
If you wish to obtain a duplicate identification card, there is a $5.00 fee. Please submit requests in writing with fee attached. Duplicates are usually sent out within two to three business days. If you wish to obtain a duplicate wall certificate, there is a $25.00 fee. Please submit requests in writing with fee attached. Duplicate certificates usually take six to eight weeks to process.
If you wish to add hearing aid dispensing to your Audiology or Provisional Audiology license, please submit a request in writing, with a $30.00 fee. This request must be approved by a Board Member. This process usually takes one to two weeks.
Individuals must remain in supervision until the license is upgraded. Notification will be sent via first class mail. Completion of supervision requirements for the ASHA Clinical Fellowship Year (CFY) does not release the licensee from the supervision mandates imposed by his/her current level of licensure.
SLP Assistant licensees wishing to convert to a Provisional Speech-Language Pathologist license must submit a new application, application fee of $125.00, Master’s Degree transcript, proof of 400 clinical practicum hours.
